Why LiveWell?

LiveWell Institute of Rehabilitation Medicine at Poovanthi, Sivagangai district situated 23 kms from Madurai has continuously provided services in the areas of stroke and other neurological conditions, brain injury and spinal cord injury since 2011. During this period, the Centre has consistently increased its capability in improving the functional independence at the time of discharge and beyond.
The centre is housed in an expanse horticultural plot with coconut and mango trees. This pollution free environment coupled with state of art facilities made available under one roof offers the patients an environment away from hustle of the city
While acute care centres treat only the illness of the patient, we, in our rehab centre, attend to the transformation of patient by providing a holistic approach that takes into consideration the physical, mental and emotional dimensions of the health of the patients.
We understand the patients’ need to return to their home and community and lead full and satisfying life. Our mission is to provide all necessary physical, social, emotional, vocational skills and to enable the patient reach highest possible level of independence, quality of life and also integrate back into their family and community. We achieve our objective by providing intimate and personalised attention, quietness of mind and sense of independence to handle one’s self

Our Guiding Principles

Our care team brings together a diverse range of skills and experience for the single purpose of helping to rebuild lives following serious and often catastrophic injury and illness. As a basic part of that goal, we believe that the uniqueness of each patient is best served by an equally unique approach to care. This belief is built into the highly personalized care plans that help direct the rehabilitation of each patient. It's also seen in the team of physiatrists, other physician specialists, nurses, therapists, psychologists, social workers, case managers and other specially trained health professionals. This team approach provides consistency during rehabilitation as well as the flexibility to quickly change personnel and resources as the patient's needs change with the recovery process. On the most basic level, the highly individualized focus on each patient is an ongoing part of our staff training and a defining feature of the culture of personalized care that has been nurtured at our Rehabilitation Centre.

Team Approach

An important factor for getting successful outcomes in rehabilitation is to have a multidisciplinary team. Team is made up of physiatrist, rehab nurses, therapists, social worker/case manager, with additional specialist staff added as care progresses or to meet specific needs.

We practice holistic 'patients centred' and “goal directed” approaches. Our personalized treatment plan addresses:

Team Approach to Care

We adopt the multidisciplinary team approach to neuro-rehabilitation, and continue to develop innovations for the treatment of brain injury, including stroke. The team-oriented approach to care is led and coordinated by certified physiatrist (physician who specialize in physical medicine and rehabilitation), along with other physician specialists including neurologists, nurses, therapists, social workers, case managers, psychologists and other health professionals with specialized training in stroke.

Throughout the inpatient experience and beyond, the most important members of this team are the individuals with stroke and their families who are encouraged and supported to take part in every aspect of care. We closely involve friends and family in support groups, recreational activities and outings, as well as in the rehab process itself, including early goal-setting and frequent team meetings

Staff of our Rehab Service

Our team consists of Physiatrist,Physio Therapists,Occupational Therapists,Speech Therapists,Nursing Staff,Social Worker,Facilities Provider and Administrative Staff.

The status of our centre

Our Rehabilitation Centre is 37-bed full-service physical rehabilitation facility which will shortly be upgraded into a 100-bed capacity centre. The quality of service rendered, the procedures and processes followed, the depth of care given to patients are on par with any leading rehab centre.

Specialization

The highly specialized staff and facility needed for treating complex medical conditions like Stroke, Spinal cord injury, Brain injury and other Neurological disorders are available at our centre.

Our centre has treated more than 1200 patients, and currently manages an inpatient volume averaging 300 patients per year. This large number of patients has helped us gain valuable experience to provide effective treatments

Enthused by the response from the patients and the outcomes from a clinical angle plans are afoot to increase the inpatient capacity from the current 37 to 100.

Integrated Rehabilitation

Individualized Programs
Even when patients have the same diagnosis, their treatment plans can vary considerably. That's why it's important to develop each treatment plan based on a range of medical and personal factors, age, goals, gender and other characteristics of each particular patient. Our Comprehensive range of services are directed toward setting up completely individualized rehabilitation programs for each patient. In addition, the recovery and rehabilitation process is also flexible enough to adapt to changing conditions during the course of treatment.

Peer Support

The most effective rehabilitation programs encourage and facilitate interaction between patients, especially those with similar diagnosis.

This network brings together current patients, families and friends and is focused less on clinical criteria and more on the physical, emotional and psychological connections that develop between people with shared experiences.

Referrals and Admissions

Livewell Rehabilitation Centre treats patients with Stroke, Spinal Cord Injury, Ortho, Brain Injury and other neurological disorders.

In addition to patients walking in and getting registered on their own, doctors in their individual capacities and also those attached to hospitals refer patients to us. These doctors make available to us histories of the patient and their current condition. Based on those reports followed by the checks and assessments which we make at the time of admission, we formulate individualized treatment plan

For those getting admitted on their own, interpreting admission criteria can be difficult, and the rules can change, so we welcome your phone call with a referral, or with general questions so that we can help provide answers.

Rehabilitation Outpatient Clinic/consultation

This Section is also located within the same campus. This clinic provides care specific to your rehabilitation needs and includes education and ongoing rehabilitative medical care.

Outpatient Therapy Services

As an outpatient, you may receive Physical Therapy, Occupational Therapy and Speech Pathology. We also provide peer and family support groups. The outpatient staff is in regular communication with our inpatient therapists to make sure that successful treatment techniques are used and continued.

Inpatient Therapy services

Livewell’s Comprehensive Inpatient Rehabilitation Program provides a broad range of therapeutic services to meet the individualized strengths, age requirements, abilities and preferences for each person served. Typical diagnosis treated in the program include major multiple trauma, orthopaedic disorders, Guillian Barre Syndrome, neuromuscular diseases, multiple sclerosis, polyneuropathy

Referrals are accepted from all sources which may include hospital and physician/group.

Bed sore Management, Transfer Technique, Personal Hygiene, Bowel and Bladder Management, Stress Management, Awareness of Rehababilitation centre, Home plan
